David Banaszak age 57 of Chicago formerly of Cleveland, Bartlett and Glendale Heights; Beloved husband of Karen (nee Miklus); Loving father of Michael and Katie; Dear son of the late Henry and Anne Banaszak; Dear brother of Robert (Cynthia) and Barbara (Steven) Misiak; Dear uncle of Rob(Megan), Christy, Jacqueline, Jennifer and Kelly. Funeral Monday, 9:30 a.m. from Salerno’s Rosedale Chapels 450 W. Lake St. Roselle, IL. 60172 (3/4 mile west of Bloomingdale/Roselle Rd.) to St. Matthew Church. Mass 10:30 a.m. Interment Graceland Cemetery. Visitation Sunday 3:00-8:00 p.m. at the funeral home. Donations may be made to the C.F.U. Scholarship Fund in David’s Memory.
